Program Highlights:
- Serve as primary care for teen parent(s) and their child (0-18 months)
- Infant well visits will be paired with a teen parent(s) visit throughout the infants’ 18 months to ensure timely comprehensive medical care for both teen parent(s) and their child
- Both infant and parent(s) can be seen separately for acute visits
- Utilize a standard parent needs assessment at the initial visits to evaluate for medical, educational/career, behavioral health, and other psychosocial stressors/social determinants of health
